Insurance Information
Our insurance specialists deal with many different insurance companies and policies. Due to the broad nature of our practice, some procedures fall under dental insurance while others may fall under medical coverage. Sometimes, procedures may be a mix of both dental and medical coverage. While we make every effort to provide you with the finest surgical care and the most convenient financial options, it is not always possible to accurately estimate our patients’ insurance co-payment. Many insurance companies do not give out fees until several months after the treatment is completed.
As a courtesy, we will file your insurance claims for you. Upon receipt of an insurance payment, any balance due will be billed to you (or a refund, if the insurance pays a higher proportion of the fee than expected).
Please be sure to bring your medical and dental insurance information with you at the initial consultation. We are in network with the following companies:

- Guardian
- Cigna
- Principal
- United Concordia
If we are not contracted with your insurance, the fees are set by the office and are the “usual and customary rates” (UCR). The fees reflect our commitment to the quality of care our patients deserve and are considered usual and customary for the area, regardless of any insurance company’s determination.
